A grandson of a lady came without a notice to appear
to the Municipal Authority of Jihlava,
the Department of Social Affairs to tell you the following information.
His grandmother is 87 years old, has been
widow for 20 years, lives alone in an apartment
1 + 1, fourth category, without sanitary facilities (it is in
the hallway). In the place of residence, there live no relatives. He lives in a
city 20 km
far from the residence of his grandmother and is currently very busy
working, does not have the possibility
to visit his grandmother every day. Grandma has been using the help of care services and emergency care. Last month, she was operated on and is no
longer able to take care of herself.
The man is a permanent resident at Jihlava, Benešova
28, his grandmother Dlouhá Brtnice 53.

Solution:
The
grandmother could express interest in
institutionalizing to a care home for the elderly. Selection of equipment is not tied to the place of
residence or the number of
applications for admission. Suitable homes for the elderly can be found
e.g. in the register of social services,
which is publicly available on the Internet. The family should primarily focus
on contacting a home for the elderly in Jihlava (there are two – care home for
the elderly Jihlava-Lesnov and Silver terraces Jihlava), in Ždírec and in Telč.
The one interested in services of the care home has the right to be acquainted with the facilities and also to see it during
a personal visit.
After
expressing interest for admission to a home for the elderly, actual medical findings must be substantiated
and (based on the decision of the applicant) the amount of income. Home for the
elderly will then conduct a social
survey on the service by the applicant and if there is a spare capacity,
they can begin to negotiate the
conditions for admission to the facility.
The agreement includes determination of the
participants concluding it – the service provider and the user, the type of services to be provided, to what extent and where,
the amount of payment for the provided services, when the contract shall
enter into force, under what
conditions the participants may withdraw from the agreement, if the contract is concluded for a definite or
indefinite period, and the number of
copies to be issued. In conclusion, there is stated when and where it was concluded and the signatures of the participants.
If the
home did not have spare capacity,
the man’s grandmother would be registered
on the list of candidates. After releasing the spare capacity, the home for
the elderly will discover, whether she is still
interested in the provision of services, conducts an actual social survey and may begin to negotiate the terms of the
contract.
The person
interested in the service is a week before entering the home requested in writing and acquainted
with the information, which documents have to be brought to the facility (e.g.
birth certificate, health insurance card, ID card) and which things may (e.g. photographs,
TV in the room). Homes for the elderly
are fully equipped according to user needs.

The woman can apply to come to a home for the elderly in Jihlava. One is located in Ždírec.
